WHY DID I RECEIVE A NOTICE?

You received Notice as you are alleged to be members (“Class 2 Creditors”) of a putative class action instituted against the Debtor by Stephanie Hoffmann (“Hoffman”) in the United States District Court for the Southern District of New  York, who alleges the such creditors sustained damages as a result of a Data Breach  by the Debtor.

 

WHAT DO I NEED TO DO TO RECEIVE A PAYMENT?

To be eligible for a payment of $201.23, you must complete the following (a) a W-9 tax form, (b) a form of Release, and either (c) an Affidavit of Medicare Non-Eligibility to be completed, signed & notarized by Class 2 Creditors who are not Medicare eligible, or (d) a Medicare query form to be completed by Class 2 Creditors who are Medicare eligible. You may download the required forms HERE. The deadline to submit these forms is December 1, 2023.

 

WHEN DID THE DATA BREACH OCCUR AND WHAT INFORMATION WAS AFFECTED?

The network hosting relevant data was attacked on July 27, 2020, and certain model contracts may have been accessible to the attacker until August 24, 2020.

 

WAS MY CONTRACT ACCESSED DURING THE DATA BREACH?

It could not be determined which model contracts were accessed during the breach, however the settlement is being made available to all models whose contracts could have been accessed during the breach.
